All health and social care partners in Northumberland share a collective vision to join up and co-ordinate care for people living across the county.
In 2015, Northumberland was awarded ‘vanguard’ status by NHS England, after being chosen as one of eight areas across the country to take a national lead on transforming care for patients.
As part of the national NHS Five Year Forward View, health and care partners across Northumberland are working together to support the further ‘joining-up’ of services by creating a fully integrated ‘primary and acute care system’.
Over the last year, the Northumberland Vanguard has introduced some new models of care across the county to transform local services and create an NHS which is fit for the future.
This week an online survey has been launched which will run throughout August and September. Patients and members of the public are being asked to share their experiences of healthcare services in Northumberland, how easily they are able to access these services and their views on the changes that have happened.
